We also offer evaluation of vulvar, vaginal, and cervical lesions including abnormal Pap Smears.
When abnormal tissue is visualized or picked up on Pap smears, usually a colposcopic examination is indicated. This usually involves the cervix, but it sometimes can involve the vagina and/or the vulva. The area in question is visualized with a magnifying instrument called a colposcope. Vinegar is applied, and this makes abnormal tissue stand out more significantly.
If there is a concern for a precancerous or a cancerous lesion, a local anesthetic is applied and then a small biopsy is taken. If a precancerous lesion is identified with the biopsy, we can usually give a local anesthetic and cauterize or freeze or excise the abnormal tissue. This is a simple outpatient process in the office.
If the colposcopy reveals a large area of abnormal tissue on the cervix, we sometimes have to perform a LEEP excision. To do this, we give a local anesthetic and then use a wire loop with cautery to excise the tissue in question. That usually will give us an accurate diagnosis and also will usually remove all of the abnormal tissue and, therefore, be therapeutic. We are especially careful to very gentle and give a good anesthetic so there is little if any pain associated with these procedures.
